The Print Physical Fitness Test Scanner Sheets will generate a scanner sheet for each teacher to keep track of the physical fitness test scores. New horizontal scanner sheets are now available which eliminates entering information by hand onto the form. It is suggested that the new forms be utilized. To print scanner sheets, click the mouse on the Print Physical Fitness Test Scanner Sheets option. The following message will display. To print the new scanner sheet forms click the mouse on the Yes button.
The message will display to check the printer line up. Click the mouse on the Yes button to check the line up. If the printer line up is already setup correctly click the mouse on the No button and the following message will display. Enter the option for which ID will be printed and click the mouse on the OK button.
NOTE: The older scan sheets can still be utilized by this program But it is suggested that the NEW scanner sheets be utilized for the Physical Fitness Testing to include the new state required fields.
The following message box will display to select the grade level for the physical fitness scanner sheets. The default grade displayed is the grade level that the state requires to be tested. In a high school, the grade would be 9, middle school would be grade 7 and in elementary grade 5. If the grade is different, type the grade for the students taking the physical fitness test. Click the mouse on the OK button.
The following message box will display. Select the sequence that the scan sheets are to be printed and type the number of the sequence in the text box. Click the mouse on the OK button.
The following is an example of the NEW Physical Fitness Scanner Sheet. Test print one scanner sheet and verify that the slugs have printed correctly prior to printing all sheets.
